General rules

 • It is the Member’s responsibility to read and understand the terms and conditions outlined below:
 • Only one card will be issued per card holder account
• The membership card is not a credit/debit card, This is used for only as a discount card
• The Discount Card benefits only for our members
• Discounts on Executive health plans may vary
• The card must be handing for benefits, no other ID can be accepted.
• This is only a Discount Card and cannot be used as any form of credit card, guarantee card or proof of age or identity.
• The card is non-transferable and only be used at our tie up sectors
• Only the person registered for the card can use it
• Discount cards only valid for the period that is printed on the card
• You can use the discount card any time within 2 years
• You must inform the company if you change your address or any other contact details.
• No limit for using the discount card within 2 year
• The Discount Card is not insurance card and does not guarantee admission to a hospital or outpatient facility.

• The discount card must be presented to the cashier before the bill is issued.
• Company reserves the right to change their offer at any time.
• Card holder confirms she/he is legally capable of entering into this Agreement.
